Chronic Pain Physical Therapy in Manhattan & Brooklyn

You’ve done the rounds. You’re still in pain. Let’s actually fix that.

If you’re reading this, chances are you’ve already tried something, maybe a lot of somethings. A course of PT that ended before you actually got better. A chiropractor. An injection. A specialist who ran out of ideas at visit six. You’ve been told your pain is “chronic” like that’s the end of the conversation, not the beginning of a better one.

It’s not the end. It’s just a different problem than the one most physical therapists are set up to treat.

We specialize in chronic low back pain, sciatica, chronic neck pain, and complex regional pain syndrome (CRPS). These cases need more time, more expertise, and more continuity than a 12-visit insurance authorization allows. One-on-one, in your home or office, in Manhattan and parts of Brooklyn.

Why Chronic Pain Needs a Different Approach

Here’s what a lot of well-meaning providers won’t tell you: after pain sticks around for a while, it stops being purely a “tissue problem.” Your nervous system gets involved and it becomes more sensitive, more protective, and more convinced that pain means danger, even after the original injury has healed. This is called central sensitization, and it’s one of the most well-documented findings in modern pain science. It’s also almost never explained to patients, which means you end up blaming yourself, or your body, for pain that has a real, treatable explanation.

Treating chronic pain well means treating the whole picture. That includes the tissue, the nervous system, the movement patterns you’ve built up to protect yourself, and the beliefs about your body that pain has left behind. That takes time and continuity most PT models can’t give you. It’s exactly what ours is built for.

What We Treat

Chronic Low Back Pain/Sciatica
If your back pain has outlasted every treatment plan you’ve tried, you’re not alone — and you’re not out of options. We build a plan around what your spine can actually do, not just what an MRI says is “wrong” with it. Most chronic low back pain/sciatica has to do with how your nervous system, strength, and movement patterns have adapted over time. We address all three.
Chronic Neck Pain
Years of screens, stress, and guarding a painful area add up. Chronic neck pain often comes with headaches, jaw tension, and a nervous system on high alert, not just tight muscles. We treat the full picture, with a plan that fits into your actual schedule instead of asking you to rebuild your week around a clinic’s hours.
CRPS (Complex Regional Pain Syndrome)
CRPS is one of the most misunderstood and under-treated pain conditions there is and one of the conditions where the right physical therapy approach matters most. Graded motor imagery, desensitization activities, and carefully progressed movement can make a real difference, but only when it’s done consistently, by someone who understands the condition and has time to actually build trust with a nervous system that’s on edge. That’s what one-on-one, in-home concierge care is built for.

Why Concierge, In-Home Care Works Better for Chronic Pain

Chronic pain doesn’t get better through one-off appointments with whoever’s available that day. It gets better through consistency with the same clinician, who knows your history, your setbacks, and your goals, working with you over time.

  • No insurance company deciding when you’re “done.” Your treatment is driven by your progress, not a diagnosis code’s visit limit.
  • One clinician, not a rotating cast. You won’t re-explain your pain history to someone new every visit.
  • Care that comes to you. No commuting on a bad pain day. Sessions happen in your home or office, on your schedule.
  • Time to actually do the work. Sessions aren’t rushed to fit three other patients into the same hour.

FAQS

Frequently Asked Questions

How is this different from the physical therapy I've already tried?
Most PT is structured around acute injuries — a set number of visits, a standard protocol, a rotating group of providers. Chronic pain doesn’t respond well to that model. We work with one clinician, over time, with a plan built around pain science and your specific nervous system, not a generic protocol.
Do you treat CRPS?
Yes. CRPS requires a specific, carefully progressed approach and a provider who understands the condition well. We have experience treating it and work closely with your physician team throughout care.
Do you accept insurance?
We’re an out-of-network, private-pay practice. This means no insurance company limits your number of visits or dictates your treatment plan. We provide the documentation (a superbill) you can submit to your insurance for potential out-of-network reimbursement. Please check our No Surprises Act page for full billing details.
What areas do you serve?
Manhattan and parts of Brooklyn, with in-home and in-office sessions. Virtual sessions are also available if that fits your situation better.
